This screening questionnaire must be time stamped within 2 hours prior to your registered class time at the studio.

Once submitted the questionnaire is time stamped and will be kept for a minimum of 2 weeks.

 

Questionnaires must be competed by a parent or guardian 18 years of age or older (dancers 18+ can complete the form themselves)

When pressing submit you agree that you have answered all questions to the best of your knowledge at the time of submission.
